If you use the contact form on our contact page, we receive the details you enter: your name, email address, phone number (if you provide it), the subject and your message. We use them to answer your question and to arrange appointments. The form sends your message to our email inbox. If you call or email us directly, we receive what you tell us in the same way.

Please do not send medical or health details, insurance member numbers, or an emergency request through the form or by email. For a sudden change in vision or eye pain, call your doctor or seek urgent care right away.

Information collected automatically
